Symptom Tracker

Use this tracker to monitor ongoing symptoms or side effects after treatment. Regularly recording how you feel can help your healthcare team detect late effects, adjust medications, or recommend further evaluations. Be honest and consistent. This log is especially useful if symptoms come and go or are triggered by specific activities, foods, or times of day.

Examples of symptoms to track include pain, fatigue, nausea, appetite changes, numbness or tingling, brain fog, swelling, and emotional distress. Record symptoms even if they seem minor,  patterns over time can be valuable for your care team.
