Advance care planning allows you to make thoughtful decisions about your future healthcare and ensure your wishes are respected, especially if there comes a time when you're unable to speak for yourself. This process includes completing documents like a Living Will and a Healthcare Proxy:

A Living Will outlines the types of medical treatment you would or would not want in serious health situations, such as life support or resuscitation.

A Healthcare Proxy names a trusted person (often called a health care agent or surrogate) to make medical decisions on your behalf if you're unable to communicate.
These documents can bring peace of mind to both you and your loved ones, helping guide care that aligns with your personal values and preferences. It’s recommended to share copies with your healthcare team and discuss your wishes openly with your family.
